This form should be completed, in ink, using block capitals. Please also include your postcode in the address section.
Don't forget to sign and date the Death Benefit Nomination Section. Please also note that cover only commences on the applicant being contracted to work in the industry, and will cease when a member attains his/her 65th birthday.

Members wishing to pay by Standing Order
Members who wish to pay their union dues by Standing Order should complete the Standing Order Mandate. Please include the full postal address, including postcode, of your bank. (The full postal address can be found on your cheque book or on any bank statement).
The application should be accompanied with £10.00 (first month's contribution).
The Standing Order should be set to commence 28 days after the Date of Application.
The entire form must be returned to the Union Head Office for processing. The reference number will be entered by office staff, and the standing order mandate will then be sent to your bank by the OILC.

Members wishing to pay by Contribution Card
Members who wish to pay by contribution card will have a contribution card enclosed with their membership pack, and need not complete the standing order section. Your application form should be accompanied with £10 (first month's contribution).

Your application cannot be processed unless the correct remittance is enclosed.
